Research summary
Output centres on the European Prospective Investigation into Cancer and Nutrition (EPIC) and on systematic reviews of diet-disease relationships. The EPIC design paper described a multi-centre prospective cohort of 519,978 participants (366,521 women and 153,457 men, mostly aged 35-70) enrolled in 1992-2000 across 23 centres in 10 European countries with planned multi-decade follow-up for cancer incidence and cause-specific mortality; baseline data collection used a non-dietary lifestyle questionnaire and a separate dietary questionnaire [4]. Within EPIC, 359,387 participants from nine countries were analysed for the association of BMI, waist circumference, and waist-to-hip ratio with risk of death using Cox regression with age as the time variable and stratification by study centre and recruitment age, demonstrating an independent contribution of body-fat distribution beyond BMI [3]. A pooled analysis of 12 prospective US and European cohorts (combined ~1.44 million adults) used self-reported baseline leisure-time physical activity and multivariable Cox regression to estimate hazard ratios for 26 cancer types, with subgroup analyses by body size and smoking [5]. Dose-response meta-analyses of prospective studies summarized fruit and vegetable intake associations with cardiovascular disease, total cancer, and all-cause mortality [2] and whole-grain intake associations with cardiovascular disease, total cancer, and cause-specific mortality, drawing on 45 studies (64 publications) identified through PubMed and Embase searches to April 2016 [6]. The 2008 World Cancer Research Fund / American Institute for Cancer Research second expert report compiled evidence on food, nutrition, physical activity, body composition, and cancer risk and specified recommendations expected to reduce cancer incidence [1]. Methodologically the work combines large multinational prospective-cohort design with pooled analyses and dose-response meta-analytic synthesis.
